Chapter 52 Home and Community-Based Services
Purpose of Life provides Chapter 52 Home and Community-Based Services to support individuals who need assistance in their daily lives. These services help clients receive care in a home or community setting while promoting independence, dignity, and personal choice.
Services may include support with:
- Daily living activities
- Personal care needs
- Community participation
- Maintaining independence at home
- Safety and supervision
- Support with routines and personal goals
- Assistance based on the client’s approved care plan
ODP Services
Purpose of Life also provides services through the Office of Developmental Programs, supporting individuals with intellectual disabilities, and developmental disabilities
Our ODP services are person-centered and focused on helping individuals live meaningful everyday lives. We support each person’s choices, goals, relationships, independence, and participation in the community.
ODP services may include support with:
- Building independent living skills
- Participating in community activities
- Supporting health and safety
- Following individual service plans
- Helping individuals work toward short-term and long-term goals
- Supporting communication, choice, and personal routines
- Working with families, supports coordinators, and team members
Flexible Care Options
Purpose of Life can provide care based on the needs of the client and family.
We can provide:
- Temporary or long-term care
- 24-hour care
- Live-in care when appropriate
- Four-hour visits
- Holiday care
- Weekend care
- A 24-hour on-call care coordinator

Getting Started with Care
When Purpose of Life begins providing care, we take time to help the client and caregiver become acquainted. Your comfort, trust, and security are important to us.
We want each client to feel respected and comfortable with the caregiver providing support. Building a positive relationship is an important part of quality care.

Private Duty Services
Purpose of Life is also available for private duty services. Private Duty clients are welcome.
